ACC and billing-related information (e.g. claim numbers, payment details)

Directly from you via intake forms, consultations, and communications
From ACC or other health professionals (with your consent)
You will be informed why the information is being collected and how it will be used.

Provide safe and effective physiotherapy care
Manage appointments and clinical records
Support ACC claims and reporting
Comply with legal and professional obligations
5. Use and Disclosure
Your information will only be:
Used for the purpose it was collected
Shared with other health providers or ACC if required for your care and with your consent
Disclosed when legally required or in cases of serious threat to life or health
We will not sell or use your information for marketing without your permission.

Paper records are locked
Digital records are encrypted and password protected
Access is restricted to authorised staff only
We take all reasonable steps to protect your information from loss, misuse, or unauthorised access.

You may request access to or correction of your personal or health information at any time by contacting our Privacy Officer.
Requests will be responded to within 20 working days, as required by law.
8. Retention and Disposal
We retain information only for as long as necessary to fulfil the purpose it was collected for, or as required by law (typically 10 years). After this, it will be securely destroyed.
